Atrial

fibrillation is common in patients with heart failure (HF) and its presence confers major prognostic implications. However, effective management of these patients is still under development. The objective of this review is to provide an overview of atrial fibrillation in HF, and the implications for management.

Recent findings

At present there is no evidence that a rhythm control strategy may bring any long-term benefits in routine management of atrial fibrillation in HF. Recently, the procedure of pulmonary vein isolation has been shown to provide clinical and prognostic benefits in patients with atrial fibrillation and concomitant HF who remain symptomatic despite optimal medical therapy, including rate and rhythm control. A meta-analysis of patients with atrial fibrillation and symptomatic HF who had implanted cardiac resynchronization therapy devices and performed atrioventricular node ablation showed significant reduction in all-cause and cardiovascular mortality, and improvement in HF functional class.

Summary

A cornerstone of management is stroke prevention, and anticoagulation is required for the vast majority of patients with atrial fibrillation and HF.

“Atrial fibrillation (AF) is the most common arrhythmia encountered by generalists and cardiologists alike. Much of the attendant morbidity from AF arises from systemic embolic complications which are effectively reduced with utilization of anti-platelet and/or anticoagulant therapy. The systemic embolic complications of AF and the medical therapy to attenuate these risks are very well established. Through the course of this review, we aim to highlight the complex relationship between AF and other, non-embolic outcomes. The presence of AF has been demonstrated to be associated with a 1.5 to 2-fold increase in mortality across numerous observational cohorts. Still further, AF frequently coexists with heart failure, whether as a causative factor or a consequence of underlying structural heart disease or neurohumoral derangement, where its presence is associated with worse clinical outcomes. Whether AF is an independent risk factor for acute coronary syndromes (ACS) remains controversial, though its occurrence in patients with ACS has been shown to be associated with adverse outcomes both in observational cohorts as well as clinical trial populations.

Patients with chronic kidney disease (CKD) are likely to have low levels of 25(OH)D, and recent observations have linked suboptimal vitamin D status with adverse cardiovascular outcomes, inflammation, insulin resistance, and the rate of progression of renal insufficiency. Little is known about the magnitude of vitamin D deficiency in pediatric patients with stage 5 CKD on chronic dialysis.

“We describe a 61-year-old patient with clinical evidence

of limbic encephalitis who improved with anticonvulsant treatment only, that is, without the use of immunosuppressive agents. Three years following occurrence of anosmia, increasing memory deficits, and emotional disturbances, he presented with new-onset temporal lobe epilepsy, with antibodies binding to neuronal voltage-gated potassium channels and bitemporal hypometabolism on FDG-PET scan; the MRI scan was normal. This is most likely a case of spontaneous remission, illustrating that immunosuppressive therapy might be suspended in milder courses of limbic encephalitis. It remains open whether treatment with anticonvulsant drugs played an additional beneficiary role through the direct suppression of seizures or, additionally, through indirect immunomodulatory side effects.
